CHV1 Forms
Here you can find guidance on filling out CHV1 forms, which will need to be completed if there are any changes to details following the Annual Parochial Church Meeting.
All UK charities, including churches, must be registered with HMRC to claim tax back on eligible donations made through Gift Aid and the Gift Aid Small Donations Scheme (GASDS).
It is the church’s responsibility to keep this registration up to date. This includes updating HMRC when any church officers or PCC members named on the registration step down from their role or leave the PCC following the APCM. Updates are made by completing a CHV1 form, which must be printed, signed with ‘wet’ signatures, and posted to HMRC.
If your church is registered for the Government Gateway, updates can also be made online. Further guidance is available on the HMRC website. You can also download CHV1 forms here (including a Gift Aid Lite version):
PLEASE NOTE: The CHV1 form is an editable PDF and must be opened correctly for it to work. If you double‑click the file and it opens in a web browser, you will not be able to complete it. Instead, save the form to your computer, then right‑click the file and select “Open with Adobe Acrobat”. This does not need to be the paid version – the free Adobe Acrobat Reader is sufficient and is already installed on most computers. Opening the form this way allows you to type directly into it and complete all fields correctly.
